Can I schedule an evaluation with Sommers Physical Therapy without a prescription from my physician?
Yes, you can schedule an evaluation and begin treatment at Sommers Physical Therapy without first having a prescription from your physician.
Please note, that under California law, you may continue to receive direct physical therapy treatment services for a period of up to 45 calendar days or 12 visits, whichever occurs first, after which time a physical therapist may continue providing you with physical therapy treatment services only after receiving, from a person holding a physician and surgeon’s certificate issued by the Medical Board of California or by the Osteopathic Medical Board of California, or from a person holding a certificate to practice podiatric medicine from the California Board of Podiatric Medicine and acting within his or her scope of practice, a dated signature on the physical therapist’s plan of care indicating approval of the physical therapist’s plan of care and that an in-person patient examination and evaluation was conducted by the physician and surgeon or podiatrist. With your written authorization, your physical therapist shall notify your physician and surgeon, if any, that he/she is treating you.
When should I schedule an in-person examination and evaluation with a physician, surgeon, or podiatrist?
We highly recommend scheduling an appointment with one of these providers as soon as you begin treatment with Sommers Physical Therapy. The passage of 45 days or 12 visits occurs quickly and many of these providers have full schedules. Please plan ahead in order to avoid disrupting the course of your physical or oral motor therapy.
Does Sommers Physical Therapy accept medical insurance?
Sommers Physical Therapy does not accept medical insurance.
Your medical insurance company may reimburse you for Sommers Physical Therapy service fees depending on your physical therapy out-of-network benefits. This will depend on your medical insurance company and your individual plan. If your out-of-network benefits are unknown, it is best to contact your insurance company for further information.
If your insurance plan allows for reimbursement and upon request, a superbill will be provided per service date. A superbill is an itemized receipt that you may send directly to your medical insurance provider for reimbursement.
